5. Accident Report Form
6. Form AA (Police Original)
7. Post Mortem Report
8. Advance Cause of Death Certificate
9. Driving License of Offending Driver
10. Insurance Policy of Offending Driver
11. Documents of Offending Vehicle :
a. R.C.T.C.
b. Fitness Certificate
c. Permit Certificate
12. If the deceased is also driving his another vehicle
Then following documents is required:
a. R.C.T.C. of Applicant’s Vehicle
b. M.D.L. of Applicant’s
c. Fitness Certificate
d. Permit Certificate
13. Name, Address, Occupation, Age of all the Applicants/Legal Heirs
14. Age Proof of deceased
15. Income Proof of Deceased
16. Hospital Bills, if any
17. Discharge Card, if any
18. Ration Card of the Applicant
19. Identity Proof of the Applicant i.e. Adhar Card/ Pan Card/ Voter Id etc.
20. Relationship of the Applicant’s with Deceased
21. One Passport Size Photos of all Applicants
22. How Accident Occurred.
